Registration and payment must be received by the noted registration deadlines to take advantage of the respective rates. 

More information about the State Capital Group's 2008 Annual Meeting will be presented following completion of this form. You will receive an email registration confirmation within five (5) business days of receipt. NOTE: Registrations must be received no later than September 23, 2008 for registrant's name to appear on the pre-printed registration list.
